About the Role

At South Shore Health, we are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. This is an exciting opportunity for a dedicated and compassionate nurse to work with patients and their families to provide high-quality care and support.

Responsibilities
  • Develop and implement individualized plans of care for patients, taking into account their unique needs and goals.
  • Collaborate with healthcare teams to ensure seamless coordination of care and effective communication among providers.
  • Provide education and support to patients and their families on health-related topics, including disease management and prevention.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
  • Participate in quality improvement initiatives to enhance patient care and satisfaction.
Requirements
  • Current RN licensure in Massachusetts.
  • Minimum 1 year of experience in acute care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Basic Life Support (BLS) certification required.
  • Advanced Cardiac Life Support (ACLS) certification may be required depending on department.
