Job DescriptionWe are seeking highly skilled Nurse Practitioners to join our team as Home Health Risk Assessment Specialists. As a member of our team, you will be responsible for conducting comprehensive health assessments in patients' homes, identifying potential health risks, and providing personalized recommendations for care.

Responsibilities- Conduct comprehensive health assessments in patients' homes, identifying potential health risks and developing personalized plans for care.
- Provide education and support to patients and their families to help them manage chronic conditions and prevent hospital readmissions.
- Collaborate with primary care physicians and other healthcare professionals to ensure seamless coordination of care.
- Participate in quality improvement initiatives to enhance the effectiveness of our health assessment service.
- Masters degree in Nursing or related field required; DNP or Ph.D. preferred.
- Current RN licensure and NP certification in good standing.
- At least 1 year of experience as a Nurse Practitioner in a home health or community-based setting.
- Excellent communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
- Able to travel up to 75% of the time, with valid driver's license and reliable transportation.
- Competitive hourly rate: $90.00 per completed assessment.
