**Job Summary:**
As a Nursing Care Coordinator, you will be responsible for implementing an interdisciplinary process to evaluate the options and services required to meet an individual's healthcare needs.

* Develop and implement individualized care plans to ensure seamless transitions and optimal patient outcomes
* Collaborate with healthcare teams to identify and address patient needs
* Conduct thorough assessments and evaluations to determine the most effective care options
* Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care
* Stay up-to-date with industry developments and best practices in care coordination
**Requirements:**
* Graduate of an accredited school of nursing, BSN recommended
* Minimum 2 years' experience in nursing and 1 year in case management, utilization management, or discharge planning
* Strong communication and interpersonal skills
* Ability to work in a fast-paced environment and prioritize multiple tasks
