About the Role

As a Clinical Team Leader with Enhabit Home Health & Hospice, you will play a crucial role in ensuring the overall coordination of home health services to all clients is delivered according to acceptable standards of practice. This involves assisting the Branch Director with day-to-day office and staff management related to patient care, as well as providing oversight of patient care and facilitating relationships between physicians, referral sources, patients, caregivers, and employees.

Key Responsibilities

  • Assist the Branch Director with electronic review and approval of patient information submitted by Licensed Professionals during Start of Care, Recertification, Resumption of Care, or Evaluation visits
  • Contribute to case conferences, reviews, and approvals of orders
  • Provide ongoing education and training to branch clinicians to ensure understanding of documentation requirements to meet regulatory standards
  • Facilitate relationships between physicians, referral sources, patients, caregivers, and employees to ensure seamless delivery of home health services

Requirements

To be successful in this role, you will need to possess:

  • A graduate degree from an accredited school of professional nursing and a valid state license
  • A minimum of two years of nursing experience, including one year in home health
  • A good understanding of Federal, State, and local laws/regulatory guidelines governing the operation of a Home Health agency
  • Skill competency with technology, including general office equipment and EMR platforms (preferably HCHB)
  • A valid state driver's license and automobile liability insurance
