Health Information Coder Specialist

Job Summary:
Cape Cod Healthcare Inc. is seeking a skilled Health Information Coder Specialist to join our team. As a key member of our medical records department, you will be responsible for analyzing and validating assigned codes based on medical record documentation.
About the Role:
- Analyze and sequence assigned codes based on medical record documentation using automated encoders, books, and coding compliance resources.
- Demonstrate complete understanding of coding rules, anatomy, physiology, and medical terminology to appropriately code patient information.
- Review all medical record documentation to determine and assign diagnoses, procedures, level codes, and modifiers to ensure accurate coding for case mix.
- Select appropriate reimbursement grouper based on financial class for each account.
- Ensure coding compliance, regulatory, and reimbursement requirements are met through assigning reimbursement classifications.
- Abstract and enter diagnosis, procedures, and level codes with demographic, clinical, and related patient information into Medical Record Abstracting and/or Billing/Accounts Receivable systems.
- Assess adequacy of documentation and query physicians and other healthcare providers to obtain additional medical record documentation or clarify documentation to ensure accurate and appropriate coding and grouping.
- Reconcile, identify, and retrieve medical records to be coded, grouped, and abstracted according to departmental procedures.
- Maintain 95% ongoing accuracy rate based on Medical Record Department performance monitors and third-party validation audits.
- Achieve weekly coding output within minimal productivity standards set by HIS, prioritizing work flow to ensure timely submission of claims and optimal productivity.
- Maintain accurate productivity logs and provide this information to the Coding Manager in a timely fashion.
- Assist in orienting and developing new coding personnel.
- Take professional responsibility for developing skills and ongoing education to maintain certification.
- Remain up-to-date on developments in health information management by pursuing professional development programs, attending educational meetings, and reviewing pertinent literature.
- Continuously monitor medical record documentation, 3M coding system, Soarian Financials system, SSI claim scrubber system, individual performance, and department workflow as related to coding function to identify problems and potential solutions.
