Job Title: Nurse Practitioner - PACE Program

Miami Jewish Health is a leading provider of healthcare and living options for aging adults in the Southeast. We operate Florida PACE (Program of All-Inclusive Care for the Elderly) Centers, the largest PACE program in Florida, which serves as a health plan, healthcare provider, and social center designed for older adults with complex medical needs.

Job Summary

Under the general supervision of the medical director of FLORIDA PACE CENTERS, the Nurse Practitioner provides direct primary medical care treatment and counsel to all participants enrolled in the program.

Key Responsibilities
  • Performs comprehensive history and physical on new referrals, and interval history and physical on established participants as indicated.
  • Manages patients with acute, episodic, and chronic illnesses.
  • Assumes ongoing responsibility for each patient under their care.
  • Refers participants to medical director or medical specialist as indicated.
  • Admits participants to the hospital, provides primary care in conjunction with Medical Director, and updates the multidisciplinary team on a regular basis.
  • Manages care of participants in the nursing home through regular visits, telephone contact with nursing home staff, and admits nursing home participants to hospital when necessary.
  • Actively participates in family conferences, attends interdisciplinary team meetings, and contributes to the team.
  • Documents treatment in patient charts according to established department guidelines.
Requirements
  • Graduate from an accredited school of nursing.
  • Advanced training and certification as an Advanced Registered Nurse Practitioner (ARNP) or advanced training and licensure as a Physician Assistant (PA).
  • 2-3 years' experience preferred; minimum of 1 year specifically working with frail elderly population.
  • Current ARNP/PA license by the Florida State Board of Nursing.
  • BLS certification required; ACLS certification preferred.
  • Drug Enforcement Administration (DEA) License required.
Work Environment

The Nurse Practitioner will work in a normal working condition with adequate lighting and ventilation. The position may involve exposure to toxic or caustic chemicals, bio-hazardous waste, and flammable, explosive gas. The ability to assist transferring patients and ambulation may involve lifting, pushing, and pulling of patients.
