We are seeking a dedicated and compassionate Registered Nurse to join our team as an Afterhours Palliative Care RN. As a key member of our Advanced Illness Management (AIM) program, you will play a vital role in providing personalized care and support to patients and their families navigating advanced illnesses.

About the Role

This is a safety-sensitive position that requires strong clinical skills, excellent communication, and a deep understanding of palliative care principles. You will be responsible for identifying and coordinating patient needs, assessing eligibility for palliative services, and collaborating with a multidisciplinary team to ensure comprehensive patient care.

As an Afterhours AIM Program - PRN RN, you will assume afterhours coverage for designated teams, utilizing your assessment, intervention, and teaching skills to maximize patient comfort and enhance quality of life.

**Responsibilities:**

  • Triage patient needs over the phone until a nurse can provide an in-person visit
  • Respond appropriately and with urgency to crisis calls from patients and families after hours
  • Make the initial nursing evaluation in determining eligibility for palliative services within 48 hours of referral
  • Collaborate with the patient/family, attending physician, and other members of the IDG in providing patient and family care

**Requirements:

  • Current RN license to practice in the state
  • CPR Certification
  • Associates/Bachelor's degree Nursing
  • 2 years of prior skilled Registered Nurse experience
