Service Coordinator and Care Navigation Specialist

Job Overview:
We are seeking a skilled Service Coordinator to oversee the delivery of high-quality services to individuals with intellectual disabilities. As a key member of our team, you will be responsible for coordinating care plans, supervising staff, and ensuring the overall quality of our services.
About the Role:
This is a full-time position that requires strong organizational skills, excellent communication abilities, and a compassionate approach to working with vulnerable populations. You will work closely with clients, their families, and other service providers to assess needs, identify resources, and develop effective solutions.
Responsibilities:
- Coordinate the development and implementation of individualized service plans
- Supervise and train Direct Support Professionals to ensure high-quality care delivery
- Liaise with healthcare providers, community agencies, and other stakeholders to optimize client outcomes
- Conduct quarterly reviews to summarize service progress and adjust plans as needed
- Participate in program meetings, training sessions, and other activities to maintain knowledge and best practices
- Mentor new staff members and support onboarding processes
Benefits:
- $62,000 - $75,000 per year, depending on experience

Requirements:
- 2-3 years of experience in direct support, case management, or a related field
- A high school diploma or equivalent; some positions may require a bachelor's degree
- Strong understanding of relevant regulations, policies, and procedures

- Ability to assess needs, identify resources, and develop solutions
