**Job Title:** Admissions Nurse Coordinator
We are seeking a dedicated and compassionate Admissions Nurse Coordinator to join our team at Avamere at Pacific Ridge. In this role, you will play a vital part in ensuring a smooth admissions process for our residents and their families.

Avamere at Pacific Ridge is a 102-bed skilled nursing community specializing in serving seniors with dementia, cognitive impairment, and psychiatric conditions. Responsibilities:
- Oversight of the admissions process for new and readmitted residents
- Cordination with department heads to ensure seamless transitions
- Ensure resident rooms are clean, safe, orderly, and comfortable upon arrival
- Be the first point of contact for resident family members, provide facility tours, and answer questions
- Conduct and coordinate resident assessments upon admission
- Initiate resident care planning with nursing and all necessary departments and personnel
- Alert department heads of any admissions changes pertinent to their department
- Collect necessary information and paperwork to complete resident admission and documentation
- Complete required recordkeeping in regards to your role
- Participate in facility surveys by authorized government agencies
Requirements:
- Nursing degree from an accredited college or university
- Current, unencumbered, active license to practice as a RN in this state
- Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities
- Experience with Electronic Medical Records and computer documentation systems
- Maintain confidentiality of all resident care information in accordance with HIPAA guidelines
