About the Role

We are seeking a skilled Registered Nurse to join our Neuro Trauma Unit as a Nurse Care Coordinator. This is an exciting opportunity for a compassionate and detail-oriented professional to make a meaningful impact in the lives of our patients and their families.

Job Summary

The Nurse Care Coordinator will be responsible for coordinating patient care, ensuring seamless transitions between departments, and providing exceptional customer service. The ideal candidate will have strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Responsibilities
  • Coordinate patient care across multiple departments, ensuring timely and efficient transfer of information
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Provide education and support to patients and families, promoting health literacy and self-management
  • Maintain accurate and up-to-date patient records, adhering to regulatory requirements
  • Participate in quality improvement initiatives, identifying areas for enhancement and contributing to process improvements
Requirements
  • RN licensure in Pennsylvania or eligibility for multistate RN license through the interstate Nurse Licensure Compact
  • BLS certification prior to orientation completion
  • 1-2 years of experience in a healthcare setting, preferably in a critical care or emergency department
  • Excellent communication and interpersonal skills, with ability to work effectively in a team environment
  • Demonstrated proficiency in electronic medical records and other relevant software systems
