Public Health Advocate for Rural Communities

About the RoleWe are seeking a skilled Community Health Worker to join our team at Healthy Oxford Hills. In this role, you will work closely with individuals, families, and community partners to provide outreach, public education, and referrals. You will also assist patients in accessing health resources and services, supporting disease self-management, and coordinating culturally appropriate care.
Key Responsibilities- Develop and implement community outreach programs to promote healthy behaviors and improve access to healthcare services
- Provide education and support to patients and their families on health-related topics, including disease management and prevention
- Collaborate with healthcare providers and community organizations to ensure seamless care coordination
- Represent MaineHealth in the community, building relationships and promoting our mission
To be successful in this role, you will need:
- A high school diploma or equivalent required; two years of experience working in a social service or healthcare setting preferred
- Valid driver's license and clean driving record (if driving is required for the role)
- Good knowledge of local resources and community assets
- Basic computer skills preferred
MaineHealth offers a competitive salary range of $45,000 - $60,000 per year, based on experience.
