We are seeking a highly skilled and compassionate Certified Hospice Nurse Specialist to join our team at Providence Home and Community Care (HCC) in Olympia, WA.

About the Role

This is a full-time position with a salary range of $38.90 - $60.54 per hour, with opportunities for additional compensation such as shift differentials, standby/on-call pay, overtime, premiums, extra shift incentives, or bonus opportunities.

The successful candidate will have a graduate degree in nursing and be certified in hospice care. They will work collaboratively with the interdisciplinary hospice team to provide high-quality patient care and enhance the quality of life for dying patients.

Responsibilities
  • Develop and implement individualized care plans for patients receiving hospice care.
  • Conduct regular assessments and evaluations to ensure optimal patient outcomes.
  • Collaborate with other healthcare professionals to coordinate care and services.
  • Provide education and support to patients and their families.
  • Participate in ongoing quality improvement initiatives to enhance patient care and services.
