About the Role

We are seeking a skilled Licensed Clinical Social Worker to join our team at Rehab Without Walls Neuro Rehabilitation. As a key member of our therapy team, you will play a vital role in delivering high-quality care to our patients.

Responsibilities
  • Provide direction, supervision, and evaluation of medical social services to patients in their homes, ensuring compliance with state practice acts and our policies and procedures.
  • Instruct, treat, observe, and evaluate patients with significant social and emotional problems affecting their health status.
  • Assist physicians and other healthcare team members in understanding the social and emotional factors related to patients' health problems.
  • Participate in the development and periodic re-evaluation of the physician's Plan of Treatment for patients requiring social work services.
  • Utilize special skills and judgment to resolve social and emotional responses connected with illnesses.
  • Observe, record, and report changes in patients' condition and response to treatment to the Clinical Manager and physician.
  • Participate in discharge planning and instruct/counsel patients and families in treating and coping with social and emotional responses connected with illnesses.
  • Provide ongoing assessment of patient and family needs and responses to teaching.
  • Participate in care coordination activities and act as a resource to other healthcare team members in identifying and resolving patient needs.
  • Utilize appropriate community resources to achieve identified objectives.
  • Supervise, instruct, and evaluate the performance of the Social Work Assistant (BSW) to ensure that all medical social services are provided to patients in compliance with Company, government, and professional standards.
  • Maintain and submit documentation as required by the Company and/or facility, including case conferences, patient/physician/community contacts, visit reports, progress notes, and confers with other healthcare disciplines in providing optimum patient care.
  • Attend, participate in, and/or conduct internal staff development programs, obtain continuing education as required by Company policies and regulations.
  • Maintain and enhance clinical practice skills.
  • Maintain the highest standards of professional conduct in relation to information that is confidential in nature.
  • Share information only when the recipient's right to access is clearly established and the sharing of such information is in the best interest of the patient.
  • Participate in Performance Improvement activities as assigned.
  • Communicate information that enables data collection and root cause analysis to identify opportunities for improvement.
Requirements
  • Bachelor's Degree required; Master's Degree preferred from a school of social work accredited by the Council on Social Work Education with six months' social work experience in a healthcare setting.
  • Current licensure/certification in the state(s) of practice if required.
  • CPR certification required.
  • Successful completion of any/all Company required pre-employment competency testing and health clearance prior to assignment and periodically as required by Company policy and state regulations.
  • Good organization and communication skills and ability to function with minimal supervision also required.
