Job Overview:

About the Role:
This challenging position involves providing quality case management and psychotherapeutic services to diverse patient populations, including adults, adolescents, children, and their families. As a key member of our interdisciplinary team, you will be instrumental in supporting the organization's treatment program and philosophy.

Responsibilities:
  • Conduct comprehensive psychosocial assessments, documenting patients' histories and identifying preliminary issues for treatment focus.
  • Deliver group and family process-oriented therapies, as well as educational and didactic groups, using various professional treatment modalities.
  • Daily progress notes summarize patients' progress toward treatment goals, including contact with family members and community agencies.
  • Develop and coordinate individualized discharge plans, utilizing treatment team and chart information to determine aftercare needs.
