We are seeking a compassionate and experienced Hospice Chaplain to join our team at Queen City Hospice. This individual will provide spiritual care and bereavement counseling to patients and their families, ensuring a patient-centered approach to end-of-life care.
Key Responsibilities- Spiritual Care Assessments: Perform timely and thorough spiritual bereavement assessments to identify patients' and families' needs.
- Direct Spiritual Care: Provide direct spiritual care and bereavement counseling to patients and their families, ensuring a supportive and non-judgmental environment.
- Exceptional Service: Demonstrate exceptional service to patients, families, referral sources, and colleagues, ensuring a high level of satisfaction and trust.
- Documentation: Accurately and timely document patient services to reflect the plan of care.
- Interdisciplinary Collaboration: Participate in group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement.
- Education and Support: Provide consultation, education, and support to the interdisciplinary group on spiritual care and bereavement counseling.
- Clinical Experience: Previous hospice experience working with death and dying is strongly preferred.
- Education: Graduate of an accredited seminary or school of theology, and/or a certificate of ordination or commission from a religious faith group.
- Interfaith Understanding: Comfortable in an interfaith setting and able to accept different lifestyles, cultures, beliefs, and values.
