As an RN Care Manager, you will:
1. Serve as the clinical bridge between professional staff across the care continuum or in the community.
2. Monitor and evaluate the effectiveness of care coordination plans, making necessary modifications.
3. Identify gaps or barriers in treatment plans and provide education to fill those gaps.
4. Conduct complete medication reviews and gather information about medication adherence.
5. Consult with medical providers regarding patient concerns and actions that may impact compliance with care plans.
6. Work closely with practices and patients regarding patient monitoring activities, adhering to assigned practice protocols.

A Day in the Life:
As an RN Care Manager, you'll perform care coordination within the scope of licensure for members with complex and/or chronic care needs. This includes assessing, developing, implementing, coordinating, monitoring, and evaluating care plans to optimize member healthcare across the care continuum.

What Success Looks Like in this Role:
1. Coordinate with patients/caregivers to ensure all physician appointments are made and kept, outpatient therapy is completed, lab/diagnostic testing is performed, and medication reconciliation is done according to protocol.
2. Track and report daily metrics and productivity measures to assess performance and support data-driven decision-making.
3. Ensure an efficient and effective holistic approach for patients/families/caregivers across the care continuum.
4. Maintain effective and professional relationships with patients and other care team members.
5. Engage patients in a therapeutic relationship and demonstrate a personal commitment to a person-centered approach, along with strong relationship-building, communication, and active listening skills.

Requirements:
We're looking for a motivated and experienced RN with a passion for population health. If you have a current, unrestricted RN license in Wisconsin (with the ability to become multi-state licensed, as applicable) and a Bachelor's degree in nursing or Associate Degree in Nursing with 3+ years in critical care, population management, or acute care experience, we'd love to hear from you. Case Manager Certification (CCM) is preferred, as is experience in Utilization Management, Case Management, Disease Management, or Discharge Planning. Prior telephonic care experience and knowledge of population health are also a plus.
