Job OverviewThis role offers a unique opportunity to leverage your expertise as a Nurse Practitioner in a PRN/Part Time capacity, focusing on Home Risk Assessments. You will conduct comprehensive health assessments, medical history reviews, and provide diagnosis and treatment plans, health education, and referrals as needed.
Responsibilities- Conduct Adult/Geriatric Assessments, medical history reviews, diagnosis and treatment plans, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as necessary.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up services.
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
- A Master's Degree or equivalent experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority.
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
- A current BLS, ACLS, or CPR certification.
- A minimum of 1 year of experience as a Nurse Practitioner.
- Strong computer skills and familiarity with Employee Health/Medical Record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Comfortable and flexible with frequent change.

The estimated annual salary for this position is $120,000, based on the location and requirements. Benefits include a mileage reimbursement plan and access to a 401(k) plan. Travel is required to meet patients in their homes, and you must have a valid state driver's license and adequate automobile insurance coverage.
