We are seeking a Licensed Vocational Nurse (LVN) to join our team at Redwood Family Care Network in Northern California.

**Job Summary:**

The LVN is responsible for assisting with the coordination of home care services to ensure residents' health and safety needs are met. This includes maintaining compliance with relevant regulations, such as the California Code of Regulations (CCR), Title 17 and Title 22, and the Lanterman Act (Welfare and Institutions Code). The LVN is expected to ensure residents maximize their independence.

Key Responsibilities:

  • Administer medications to individuals and verify the five rights for accuracy
  • Attend residents' doctor appointments and address any concerns or changes with the attending physician
  • Assist individuals with activities of daily living, including bathing, cleaning, preparing meals, grooming, cooking, and cleaning
  • Audit all medications during a new medication cycle and verify the five rights
  • Perform g-tube insertions for feeding individuals
  • Perform colostomy care for individuals
  • Perform intramuscular injections
  • Perform diabetes insulin medication injections for individuals
  • Evaluate residents' and participants' level of consciousness, neurological factors, and responsiveness post-seizure or traumatic episodes
  • Redirect individuals in the event of a behavioral episode and continue to support their needs
  • Complete and update client notes system daily. Observe and report any unusual behaviors and injuries so both Behavior Consultant and Administrator are informed
  • Assess and identify signs and symptoms of infections (early and late signs)
  • Perform basic physical reviews, including checking vital signs and conducting head-to-toe assessments
  • Participate in community outings with individuals, such as hiking, visiting the beach, and other leisure activities

Requirements:

  • 1 year of experience supporting adults with developmental disabilities
  • Licensed Vocational Nurse License (required)
  • Must have First Aid/CPR and CPI (Crisis Prevention Intervention) certification
  • Must be at least 21 years old
  • Must have a California Driver's License
  • Candidate must pass all pre-employment requirements, including a background check

Available shifts: AM, PM, NOC

Estimated Salary Range: $32.00 - $38.00 per hour, depending on experience
