Personalized Health Partners, a leading provider of healthcare services, is seeking an experienced Nurse Practitioner to join our team in the Charleston, WV region.

The ideal candidate will have a passion for delivering high-quality care to seniors and other residents in various healthcare settings. As a PHP Nurse Practitioner, you will work first shift, Monday - Friday, without on-call or weekend/holiday coverage.

We are committed to providing exceptional customer experience and satisfaction, with a focus on reducing hospital admissions and emergency room visits. Our NP collaborates with PCPs and interdisciplinary teams to develop plans of care that prioritize the well-being of our residents.

This role offers a competitive salary of $115,000 annually, based on industry standards and regional factors.

About the Job:

  • Assess, diagnose, and manage chronic and acute conditions in seniors and adults.
  • Provide personalized health education and guidance to residents and their families.
  • Develop effective treatment plans and communicate them clearly to relevant stakeholders.
  • Maintain accurate and compliant documentation within our electronic medical record system.
  • Collaborate with local and regional operations and clinical leadership to achieve business objectives.
  • Support strategic initiatives focused on value-based care and population health management.
  • Deliver excellent customer service and foster a positive relationship between residents, caregivers, and healthcare providers.
  • Prioritize tasks effectively, manage competing deadlines, and maintain attention to detail.

Requirements and Qualifications:

  • A Master's degree in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate from an accredited institution.
  • National certification as a Nurse Practitioner (AANP or ANCC).
  • An active, unrestricted state license to practice nursing.
  • A valid DEA license.
  • US work authorization.
  • At least 2 years of clinical experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice settings.
  • Ability to adapt to a flexible work environment and schedule.
