Job Summary

We are seeking a highly skilled Homecare Intake Specialist to join our team at Mai Placement in Brooklyn, NY. This full-time, in-person role offers a competitive salary of $60,000–$65,000, with significant bonus potential based on performance.

The ideal candidate will be fluent in Spanish and English, with prior experience in homecare intake and a proven track record of managing high-volume caseloads.

Key Responsibilities:

  • Manage referrals from the Business Development team and contact patients or their proxies to assess needs.
  • Coordinate all steps necessary to initiate homecare services, including Medicaid application and processing.
  • Handle a caseload of 100-150 patients, including 40-50 follow-ups monthly, ensuring seamless and efficient service delivery.
  • Provide bilingual support to patients, addressing their questions and guiding them through the intake process.

Requirements:

  • Fluent in Spanish and English (verbal and written communication).
  • Proven experience in homecare intake or a similar role, with knowledge of Medicaid processes.
  • Strong organizational skills to manage a high-volume caseload and meet deadlines.
  • Excellent interpersonal and communication skills to build trust with patients and proxies.
  • Ability to work independently, solve problems proactively, and thrive under pressure.
