Key Responsibilities- Conduct comprehensive assessments of referred members' needs and eligibility, collaborating with case managers to determine an approach to resolving participant issues and meeting needs utilizing available resources.
- Provide high-quality care while adhering to safety and agency policies, maintaining progress notes for each participant.
- Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers, addressing complex clinical indicators affecting care planning and resolution of participant issues.
- Perform crisis intervention with participants experiencing behavioral health or medical crises, referring them to appropriate clinical and service providers for thorough assessment and treatment as clinically indicated.
- Provide crisis follow-up to participants to ensure they receive appropriate treatment and services.
- Collaborate with caregivers to meet participants' needs, helping them understand treatment procedures and overcome challenges.
- Discern participants' words and actions, helping them identify root causes of their issues.
- Follow Volunteers of America Southeast Louisiana's Performance Management process, ensuring staff understand and follow policies and procedures.
- Assess risk, mental status, and medical stability of clients requiring crisis intervention and refer as needed.
- Manage a caseload of participants for therapy, performing direct clinical, diagnostic, assessment, treatment, and outreach services.
- Participate in training required by accreditation standards, NCTSI-II, and performance and quality improvement efforts, adhering to Performance, CARF, and Quality Standards.
- A Master's Degree in social work or counseling, Louisiana licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or other equivalent.
- Level of Care Utilization System (LOCUS)/CALOCUS certification, case management, and discharge planning experience, managed care, and crisis intervention skills are strongly preferred.
- Ability to communicate clearly in person and/or via telephone communication, and in writing.
- Organizational skills and the ability to prioritize multiple tasks concurrently.
- The ability to recognize own limitations and respond to difficult situations.
- Interact with appropriate agencies and persons in the community in developing services.
- Ability to generate documents and spreadsheets using computer software, including Microsoft Word and Excel.
Normal office environment, with potential for overnight travel and local travel as required for performance of duties and responsibilities.
Incumbent must have a current driver's license, vehicle inspection decal, vehicle insurance, vehicle registration, and a satisfactory Motor Vehicle Report.
