Job Summary

The Community Health Worker provides critical support to patients and community members, facilitating access to healthcare services and promoting health literacy.

Key Responsibilities
  • Recruit patients through outreach activities and build trust to provide support, education, and case management services.
  • Advise patients on healthcare services and assist with navigation, making follow-up contacts as needed.
  • Conduct assessments of health behaviors, social factors, and economic resources to inform care planning.
  • Educate patients with chronic illness on evidence-based standards of care and self-management techniques.
  • Document work with patients and assist with data collection for program evaluation.
  • Collaborate with healthcare teams and participate in clinic meetings and patient visits.
  • Serve as a liaison between healthcare staff and the community, developing relationships with stakeholders.
  • Transport patients between healthcare settings and meet with patients in community settings.
Requirements
  • Two years of experience providing information, education, and referral services to diverse populations.
  • Recent experience working in a healthcare setting, preferably with a multi-disciplinary team.
  • Community Health Worker certification in Rhode Island is preferred.
  • Excellent verbal and written communication skills, with the ability to juggle multiple priorities.
  • Demonstrated attendance and reliability, with prior experience using an electronic health record preferred.
