At AbsoluteCare, we are seeking a skilled Advanced Practice Provider to join our team as a Community Based Clinician. This role is designed to provide primary and urgent care to our members in the comfort of their own homes.

The ideal candidate will have experience in adult medicine with acute care, be certified in ACLS, and possess excellent communication skills. As a Community Based Clinician, you will work closely with our care team to ensure seamless transitions of care, particularly after hospital discharge.

Key responsibilities include:

  • Providing community-based medical and care-coordination services for recent hospital discharges
  • Managing a small panel of homebound members who require primary care
  • Resolving acute issues in the home and clinic or by directing members to the most effective and efficient solutions

Additionally, you will:

  • Ensure coordination of care to include timely and accurate completion of chart documents
  • Work with the Treatment and Triage Nurse to address sick calls and visits requiring Provider management
  • Collaborate with the Integrated Care Team (ICT) members for problem-solving and coordination of care
  • Identify and refer to specialty programs when instability or crisis occurs

Requirements:

  • Certified Registered Advance Practice Provider in a field of Adult Medicine with Acute Care experience
  • ACLS certified
  • Ability to take a creative and innovative approach to problem-solving
  • Excellent computer skills, including knowledge of Microsoft Office

Preferred qualifications include experience in a Patient Center Medical Home and working with high-risk and medically complex patients.
