Public Health Community Heart Work Member I-Full Time

We are seeking a highly motivated and compassionate Public Health AmeriCorps Community HEART Work (PHACHW) Member I to join our team at Christus Health. As a PHACHW Member I, you will play a vital role in providing screening, referral, and navigation services to connect individuals and their families to social and medical services to resolve their identified needs.
Key Responsibilities- Collaborate with a team of healthcare professionals to provide patient-centered care and develop care plans that address individual barriers and goals.
- Provide culturally competent education and information about health, wellness, and social services to patients and their families.
- Assist patients in developing action plans to address identified needs and encourage them to set goals, identify challenges, and monitor their progress.
- Link patients to community resources and services through referral and navigation services.
- Enhance care quality by aiding communication and building relationships between patients and healthcare providers and/or community resource providers.
- Consult with administrative supervisor and clinical team to coordinate activities and address identified healthcare needs of patients.
- Assist with chronic disease self-management and medication adherence.
- Ensure education about primary care and appropriate emergency department use and assist patients to identify primary care options and a medical home.
- Complete appropriate documentation and record keeping that follows organizational policies and procedures and state and federal regulations.
- High School Diploma or equivalent.
- Bilingual (English/Spanish) preferred.
- 1 year of experience volunteering, working in the community, or working in a healthcare facility, public health department, or community-based organization preferred.
- Promotor(a) or Community Health Workers Certification or willingness to obtain within six months of start.
