We are seeking a highly skilled Traveling Healthcare Professional to join our Senior Community Care (SCC) team. As a member of this interdisciplinary team, you will provide clinical coverage for our highest-risk health plan members and play a vital role in coordinating their care.

**Salary:** $88,000 to $173,200 annually

**Job Description:**

The Traveling Healthcare Professional will be responsible for providing comprehensive, monthly/routine, and/or acute care visit assessments, managing members through acute medical situations or exacerbations of chronic conditions, and ensuring accurate and complete documentation, including ICD 10 conditions.

**Key Responsibilities:**

* Provide clinical coverage for the primary APC during periods of PTO or short/long term absences

* Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions

* Manage members during a short term, sub-acute rehabilitation episode

* Conduct comprehensive, monthly/routine, and/or acute care visit assessments

* Effectively manage acute and chronic medical and behavioral conditions

* Ensure accurate and complete documentation, including ICD 10 conditions

* Communicate and collaborate with the interdisciplinary care team

**Requirements:**

* Active NP or PA license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice

* 1+ years of clinical/family/geriatric practice experience as an NP or PA

* Willingness to travel via car, airplane, etc.

* Willingness to obtain additional licensure in assigned geographic areas

* Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment

* Proficient with electronic medical records and the internet

**Preferred Qualifications:**

* Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs

* Understanding of Geriatrics and Chronic Illness
