Job Summary

We are seeking a skilled RN Care Coordinator to join our team on a per diem basis. As a key member of our primary care team, you will play a vital role in coordinating care services and supporting our members in achieving their health goals.

Key Responsibilities
  • Establish and maintain effective communication with members, caregivers, and healthcare providers to ensure seamless care coordination.
  • Review member status and develop individualized care plans to achieve progress toward goals.
  • Perform administrative tasks, such as data collection and entry, to support care coordination efforts.
  • Collaborate with healthcare providers and other team members to ensure comprehensive care planning.
  • Provide education and support to members and caregivers on health-related topics.
Requirements
  • Current unrestricted Registered Nurse license in Oregon.
  • 3+ years of RN experience, with 1+ year of case management experience.
  • Valid driver's license and ability to travel locally 75% of the time.
  • Experience with electronic medical records and Microsoft Office.
  • Excellent communication and customer service skills.
Preferred Qualifications
  • BSN degree.
  • Experience working in post-acute care, such as long-term care or hospice, and/or case management experience as an RN.
  • Experience working with the geriatric population.
  • Familiarity with the long-term care business environment.
