Intake Therapist

The Intake Therapist at Universal Health Services is responsible for ensuring a smooth, trauma-informed, and orderly process for telephonic and in-person assessments to assist physicians in identifying the appropriate level of care for potential patients.
Key responsibilities include initial contact with referral sources, potential patients, coordinating the referral and intake process, and admission process, as well as providing trauma-informed assessment/treatment of patients and maintaining communication with referral sources, families, patients, and unit staff.
This role interfaces closely with internal hospital staff, facility physicians, referring hospitals, patients, families, and community referral sources, and may also be responsible for verifying and prior authorizing insurance benefits as needed.
Requirements:
- Master's degree in Social Work or related field required.
- Licensed as an LCSW, LPC, or similar independent AZ Board of Behavioral Health licensure preferred.
- 1-3 years of experience working in a psychiatric setting.
- Must have or be able to obtain a Level 1 Fingerprint Clearance Card.
