Hospice Care Nurse

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Care team. As a RN, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.
Responsibilities
* Provide direct patient care, including administering medications, wound care, and vital sign monitoring
* Develop and implement individualized care plans to meet the unique needs of each patient
* Collaborate with interdisciplinary teams to ensure comprehensive care
* Educate patients and families on disease management, medication use, and self-care techniques
Requirements
* Current RN licensure in the State of Wisconsin
* Associate's degree in Nursing (Required)
* Valid driver's license and reliable transportation
