Medical Care Transition Specialist

We are seeking a highly skilled Clinical Transition Coordinator to join our team at Soleo Health Inc.
The estimated annual salary for this role is around $70,000-$90,000 based on national averages and the level of experience required.
About the RoleThis Clinical Transition Coordinator position plays a crucial role in coordinating patients' care transitions from hospitals or healthcare facilities to alternative sites of care.
- Sales Support and Patient Referral: Collaborate with the sales team to strategize and secure new referral sources in a geographic market. Work closely with local territory managers to gain new referrals from physicians, case managers, and other potential points of contact.
The ideal candidate will have a minimum of 5 years of clinical experience, preferably in home health or management. A Bachelor's degree is preferred, but not required.
ResponsibilitiesThis Clinical Transition Coordinator will be responsible for:
- Maintaining and implementing up-to-date coordinated care plans to transition patients to alternative sites of care.
- Communicating with multidisciplinary teams, including referring physicians, hospital case managers, and branch staff.
- Providing patient education and support throughout the care transition process.
- Documenting demographic, clinical, and payer information accurately and timely.
- Securing necessary paperwork and signatures needed to commence therapy and submit for payer reimbursement.
