At Encompass Health, we're seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you'll play a vital role in coordinating patient care from admission to post-discharge, ensuring seamless transitions and optimal outcomes.
Key Responsibilities:
- Coordinate patient care plans with interdisciplinary teams
- Monitor patient experience and quality of care
- Facilitate team conferences and treatment plan modifications
- Complete case management addendums and documentation
- Stay up-to-date on regulations, company policies, and department operations
Requirements:
- Qualifications: RN, SW, OT, PT, ST, or Rehabilitation Counseling licensure
- CCM or ACMTM certification preferred
- Minimum 2 years of rehabilitation experience
