Position Summary:

The Franklin County Board of Developmental Disabilities is seeking a Service Coordinator to join our team. As a Service Coordinator, you will work directly with individuals with developmental disabilities, providing them with the support and resources they need to thrive.

Responsibilities:

As a Service Coordinator, you will:

\
  • \
  • Provide person-centered services to individuals with developmental disabilities\
  • Coordinate services among service providers and monitor Individual Service Plans (ISPs)\
  • Write and prepare reports, correspondence, case notes, assessments, and ISPs\
  • Maintain current files, records, and other materials\
  • Attend and/or conduct meetings related to assigned caseload\
  • Consult with service providers to improve or expand existing services\
  • Participate in community engagement activities to promote awareness of service coordination\

Requirements:

  • Bachelor's Degree in Human Services or related field\
  • Valid Ohio Driver's License\
  • Ability to travel to various parts of the county\
  • Strong communication and interpersonal skills\
  • Ability to work independently and as part of a team\
  • Knowledge of disability support services and community resources\
  • Mandated reporter certification\
  • Ability to maintain confidentiality and handle sensitive information\
