Hospital Pharmacy Expert

We are seeking a highly skilled Clinical Pharmacist to join our team at Yale New Haven Health. The ideal candidate will have strong analytical and communication skills, with the ability to work effectively in a fast-paced environment.
Key Responsibilities- Assure accurate and timely medication delivery to patients by resolving drug distribution issues.
- Formulate medication monitoring plans based on clinical and pharmacokinetic lab data.
- Communicate and document problem detection, relevant findings, recommendations, and follow-up actions.
- Provide expert advice on drug use and safety to healthcare professionals.
- Participate in educational forums to enhance pharmacy services.
- Bachelor's or Doctor of Pharmacy degree from an accredited institution.
- Completion of a hospital pharmacy residency program is desirable.
- Two years of experience in an acute care hospital pharmacy setting is preferred.
- Licensure to practice pharmacy in Connecticut or eligibility for reciprocity.
The estimated annual salary for this position is $130,000-$160,000, depending on experience. We offer a comprehensive benefits package, including medical, dental, and vision insurance, retirement savings plan, and paid time off.
