Part-time Clinic Nurse Specialist

We are seeking a highly skilled and compassionate Part-time Clinic Nurse Specialist to join our team at the Los Angeles Christian Health Center. As a key member of our healthcare team, you will play a vital role in delivering exceptional patient care and promoting overall well-being.

About the Role:
  • Provide professional nursing care to patients in a clinic setting, adhering to policies, procedures, and standards of the RN license and LACHC.
  • Mentor and educate patients on health maintenance, disease prevention, and chronic care management.
  • Collaborate with multidisciplinary care teams to improve patient outcomes and promote a culture of service excellence.
Responsibilities:
  1. Nursing Care: Provide basic medical case management, communicate with patients and families, reconcile medications, provide interventions as needed, and facilitate clinical follow-up for self-management or self-care goals.
  2. Triage and Assessment: Perform triage on patients requesting same-day or urgent appointments, providing nursing intervention when necessary, and determining if additional visits are required per established protocol.
  3. Clinical Follow-up: Participate in patient follow-up activities as directed by providers for normal and abnormal laboratory, DI, or specialty consult results, interacting with patients via phone, text, letter, or email/portal.
  4. Medication Administration: Dispense medications according to approved standardized procedures, including DOT (direct observation therapy), placing TB tests, nebulizer treatments, and giving medication injections and vaccines.
  5. Documentation: Document all patient interactions accurately and thoroughly in the patient chart, ensuring complete, prompt, and compliant records.
  6. Crisis Intervention: Provide crisis intervention when required to promote a safe and peaceful clinic environment, keeping patients and staff safe.
  7. Education and Advocacy: Educate patients on chronic care, disease prevention, and health maintenance, serving as an advocate for patient needs and promoting a holistic approach to treatment.
Requirements:
  • Diploma from an accredited nursing school; Bachelor's degree (BSN) preferred.
  • One year community or ambulatory health experience required; experience in case management preferred.
  • Current, unencumbered, active California license to practice as an RN; CPR certificate.
