About the Position
Transformation House is a leading provider of integrated behavioral health services, licensed by the Minnesota Department of Human Services. We offer a unique opportunity for a Certified Nurse Practitioner or Psychiatric Nurse Practitioner to join our multidisciplinary team.

We are currently hiring for a part-time position in our substance use disorder treatment facility. As a Medical Practitioner - Substance Use Disorder Specialist, you will be responsible for providing expert client care, medication management, and referrals. You will work closely with our multidisciplinary team to ensure seamless care delivery.

Responsibilities:
  • Provide Medication Management (including suboxone)
  • Make Referrals
  • Consult with the Multidisciplinary Team
  • Document client information accurately and timely

Requirements:
  • Licensed by the Minnesota Board of Nursing as a Nurse Practitioner
  • Willing to submit to and pass a DHS Background Study
  • Basic computer skills

Estimated Salary Range:
$62,000 - $82,000 per year
