Job SummaryThe Medical Social Worker III provides assessments, coordination, and implementation/evaluation of professional social services to patients and their families in order to aid them in coping with social, emotional, and economic difficulties related to medical problems or which predispose to illness.
Key Responsibilities- Assessment and Intervention: Conduct bio-psychosocial assessments with patients, families, and significant others to identify needs and develop treatment plans.
- Crisis Intervention: Provide crisis intervention and treatment to adults, children, and families in emergent situations.
- Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, interventions, and outcomes.
- Collaboration and Communication: Work effectively with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive patient care.
- Community Resource Referral: Identify and provide patients with community resources and referrals to support their social and emotional needs.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care and services.
- Education: Master's degree in Social Work (MSW) from an accredited program.
- Experience: Minimum one year of experience working as a MSW in medical acute care setting or as part of a medical team.
- Skills: Strong assessment and intervention skills, excellent communication and interpersonal skills, ability to work effectively in a team environment.
