About the Role:

We are seeking a skilled Clinical Care Coordinator to join our team at Community Health System. This is a challenging and rewarding role that requires strong communication skills, attention to detail, and the ability to work well under pressure.

Key Responsibilities:

  • Evaluate assigned patients and develop individualized care plans to ensure optimal health outcomes.
  • Collaborate with physicians and other healthcare professionals to provide comprehensive patient care.
  • Conduct patient assessments, administer medications, and perform various tests as needed.
  • Teach patients and families about self-care skills, condition management, and medication use.
  • Maintain accurate and detailed records of patient interactions and treatment plans.

Requirements:

  • Associate's degree in Nursing or related field required.
  • Certified as a Registered Nurse (RN) with current licensure.
  • Proven experience working in a healthcare setting, preferably in a night shift role.
  • Excellent communication and interpersonal skills.
