Job Summary: We are seeking a highly skilled Patient Navigator to join our team at Alabama Regional Medical Services. As a Patient Navigator, you will play a critical role in helping patients access healthcare services and navigate the healthcare system.

Key Responsibilities:

  1. Guide patients through the healthcare system, ensuring they receive timely and effective care.
  2. Assist patients in preparing for scheduled appointments, including arranging transportation and providing support.
  3. Screen patients for program eligibility, enrollment, and consenting, and identify potential barriers to care.
  4. Develop and implement strategies to address patient barriers, including connecting patients to community and social support services.
  5. Provide education and support to patients, reducing anxiety and fear related to healthcare.
  6. Facilitate referrals for mammogram screenings and other diagnostic tests, and communicate with patients in a culturally competent manner.
  7. Assist patients in finding ways to pay for their breast and cervical healthcare.
  8. Provide women's health education to individuals and groups.
  9. Participate in internal and external meetings, as required.
  10. Track and document interventions and outcomes.
