Healthcare Access Specialist

Job Summary: We are seeking a highly skilled Patient Navigator to join our team at Alabama Regional Medical Services. As a Patient Navigator, you will play a critical role in helping patients access healthcare services and navigate the healthcare system.
Key Responsibilities:
- Guide patients through the healthcare system, ensuring they receive timely and effective care.
- Assist patients in preparing for scheduled appointments, including arranging transportation and providing support.
- Screen patients for program eligibility, enrollment, and consenting, and identify potential barriers to care.
- Develop and implement strategies to address patient barriers, including connecting patients to community and social support services.
- Provide education and support to patients, reducing anxiety and fear related to healthcare.
- Facilitate referrals for mammogram screenings and other diagnostic tests, and communicate with patients in a culturally competent manner.
- Assist patients in finding ways to pay for their breast and cervical healthcare.
- Provide women's health education to individuals and groups.
- Participate in internal and external meetings, as required.
- Track and document interventions and outcomes.

- Ability to maintain confidentiality and handle sensitive information.
