About the Role

We are seeking a dedicated and compassionate Case Manager to join our Outreach Services-TLS team at PATH (People Assisting the Homeless). As a Case Manager, you will play a vital role in assisting clients in breaking the cycle of homelessness by providing individualized supportive services.

Key Responsibilities
  • Conduct intake and individualized needs assessments for clients, collaborating to develop an Individualized Service Plan (ISP) that addresses housing goals, income, and other personal objectives.
  • Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals.
  • Complete program-specific assessments, such as the VI-SPDAT, to evaluate strengths and challenges in addressing short-term and long-term goals.
  • Maintain accurate and up-to-date client files, adhering to documentation standards and confidentiality guidelines.
  • Develop therapeutic and professional relationships with clients and systems/partners to enhance engagement and retention efforts.
Requirements
  • Two years of experience in healthcare, case management, homeless services, or a related field.
  • Excellent analytical, communication, and problem-solving skills.
  • Ability to work effectively with diverse populations and maintain confidentiality.
  • Valid driver's license and reliable transportation.
