Job Summary

We are seeking a highly skilled Clinical Care Coordinator - Registered Nurse to join our team. This role will play a critical part in supporting patients and families throughout their healthcare journey.

Key Responsibilities:
  • Coordinate patient care services to ensure timely and excellent outcomes.
  • Facilitate communication among the medical team to promote seamless care delivery.
  • Identify and address barriers to timely diagnosis and treatment, connecting patients with relevant resources.
  • Build strong relationships with patients, families, and caregivers through effective communication and listening skills.
Requirements:
  • Clinical competency as required for the position, including age-specific competencies.
  • Knowledge of nursing theory and practice, including leadership skills.
  • Ability to analyze information, understand statistics, and provide input into policy development.
  • Effective communication and interpersonal skills.
Salary Information:

The estimated salary for this position is $75,000 - $90,000 per year, depending on experience and qualifications.

About the Role:

This role requires a registered nurse with a minimum of three years of recent clinical experience, preferably in an ambulatory clinic setting. The successful candidate will be able to work collaboratively with healthcare teams to deliver exceptional patient care.
