Job Description
We are seeking a Care Coordinator for Elderly Residents to join our team. As a key member of our healthcare staff, you will be responsible for developing and implementing diverse range of activities and programs tailored to the interests and abilities of our residents.

Responsibilities
  • Activity Planning: Develop engaging activity plans that cater to the diverse range of interests and abilities of our residents.
  • Individualized Care: Assess the needs and preferences of residents to customize activity plans and ensure participation and enjoyment for all residents.
  • Group Facilitation: Lead group activities and events, encouraging resident participation and fostering a sense of community and belonging among residents.
  • One-on-One Engagement: Provide one-on-one interaction and engagement with residents who require individualized attention or assistance with activities.
  • Documentation: Maintain accurate records of resident participation in activities, as well as observations of resident engagement, behavior, and mood.
  • Collaboration: Work collaboratively with other members of the healthcare team to coordinate care plans and ensure the holistic well-being of residents.
  • Safety and Supervision: Ensure the safety and well-being of residents during activities, providing supervision and assistance as needed.

Requirements
  • Certification: Valid certification as a Certified Nursing Assistant (CNA) in the state of Florida required.
  • Previous experience working with elderly individuals, preferably in a long-term care or assisted living setting, is preferred but not required.
  • Genuine compassion for the elderly and a desire to enhance their quality of life through meaningful activities and interactions.
  • Excellent communication and interpersonal skills, with the ability to interact effectively with residents, families, and staff.

Salary Information
The estimated salary for this position is $45,000 - $60,000 per year, depending on experience and qualifications.
