Physical Therapist Care Coordinator

About the RoleThis is a full-time position that requires a Bachelor's Degree from an accredited Physical Therapy program. A Master's Degree is preferred.
- The ideal candidate will have customarily five years of demonstrated experience in an orthopedic setting with use of modalities, therapeutic exercises and isokinetic equipment.
- Ortho-outpatient experience is preferred.
As a Physical Therapist Care Coordinator, your responsibilities will include:
- Providing rehabilitative services for disabled, injured, and diseased patients within the center.
- Aligning with Concentra Medical Centers' Injury Process Management, their rehabilitation philosophy in occupational medicine, and relevant policies, practices, and regulations.
- Ensuring delivery of high-quality patient care services.
- Supporting and encouraging multidisciplinary coordination of treatment.
- Maintaining professional standards through compliance with quality assurance policies/procedures as well as applicable regulations and corporate policies.
- Implementing effective communication with own and other departments' staff, with medical staff and patients to ensure coordinated efforts and provision of high-quality service and optimal patient care.
We offer a competitive salary range of $79,436.25 to $104,521.38, depending on job-related factors such as level of experience, geographic location, and seniority.
