About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at the Augusta Center for Health & Rehabilitation. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the well-being of our residents.

Job Summary

The successful candidate will be responsible for administering medications and treatments as prescribed by healthcare providers, monitoring and recording patient vital signs and medical information, collaborating with the interdisciplinary team to develop and implement care plans, providing wound care and other nursing interventions as needed, and supervising and supporting Certified Nurse Assistants (CNAs) in delivering care.

Key Responsibilities
  • Administer medications and treatments according to physician orders.
  • Monitor and record patient vital signs and medical information.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Provide wound care and other nursing interventions as needed.
  • Supervise and support CNAs in delivering care to patients.

Compensation and Benefits
