At Optum, we're creating a new model of care that's centered around the patient's needs. As a Part Time Clinical Nurse Practitioner, you'll play a vital role in delivering high-quality, patient-centered care in the comfort of their own homes.
Key Responsibilities:
- Conduct comprehensive in-home assessments, including medical history, review of systems, medication reconciliation, vital signs, and physical exams
- Perform evidence-based practice screenings, including point-of-care testing as needed
- Identify diagnoses and develop care plans to address patient needs
- Communicate findings with patients' primary care physicians to ensure seamless care coordination
- Intervene in urgent and emergent situations, providing timely and effective care
- Educate patients on disease management, medication adherence, and healthy lifestyle choices
Requirements:
- Active NP license or ability to obtain by start date
- APCs must have approval to practice autonomously or without formal supervision
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care
- Ability to complete physical demands of the job, including lifting, navigating stairs, and sitting/standing/kneeling
- Reliable transportation for home visits
Preferred Qualifications:
- 1+ years of clinical experience in a similar setting
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home health care or home visit experience
- Ability to travel up to 5% of the time, with additional compensation incentives available
