Trinity Health is a leading healthcare provider dedicated to delivering exceptional care to patients and families in their homes. As a Registered Nurse (RN) on our Hospice Homecare Team, you will have the opportunity to make a meaningful difference in the lives of those we serve.

About the Role

We are seeking an experienced RN to join our team in the Columbia/Greene County area. In this full-time role, you will work closely with an interdisciplinary team to provide comprehensive care to patients and families in their homes.

Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Coordinate with physicians, social workers, chaplains, and other healthcare professionals to ensure seamless care delivery
  • Provide emotional support and guidance to patients and families during challenging times
  • Maintain accurate and up-to-date medical records
Requirements

To be successful in this role, you must possess:

  • A current license to practice as a Registered Nurse in the State of New York
  • An Associate's Degree in Nursing or Bachelor's Degree in Nursing preferred
  • At least one year of experience as an RN, preferably in Hospice/Home Care or acute care
  • Homecare experience is required
  • A valid Driver's License and reliable vehicle
  • Excellent communication and interpersonal skills
