About the Role

A key aspect of this role is your ability to communicate effectively with members, families, and healthcare providers. You will conduct face-to-face assessments, develop comprehensive care plans, and facilitate coordination across the continuum of services. Your expertise in geriatric care, behavioral health, and home health will be invaluable in providing holistic support to our members.
