Community Health Nurse

We are a faith-based non-profit, Federally Qualified Health Center serving the communities of Detroit in our clinics that offer integrated medical, dental and counseling healthcare services.
Job Type: Part-time, 24 hours/week
Estimated Salary Range:$60,000 - $80,000 per year, depending on experience
About the RoleThis exciting opportunity allows nurses to practice to a high degree of professional autonomy in a challenging, yet highly rewarding setting. As a Community Health Nurse at our Moross clinic, you will be part of an interdisciplinary team providing comprehensive care to underserved patient populations.
Your Key Responsibilities:
- Triage patients presenting with questions and concerns
- Conduct assessments, education, and lab ordering using established protocols
- Accurately document nursing information
- Respond to medication refill requests and provide medical advice
- Review lab results and notify patients of abnormal findings
- Function as a member of an interdisciplinary team to provide comprehensive care
- Assist in Care Coordination for patients
- Communicate effectively with community partners and staff
Requirements:
- Associate's Degree or Bachelor of Science degree in Nursing
- Current, unencumbered Registered Nurse license in the State of Michigan
- Proficiency with computers and Electronic Health Records
- Knowledge and experience using EPIC is a plus
