Job Summary

Acclaim Multi-Specialty Group and JPS Health Network are seeking an experienced Advanced Practice Provider to join the Community Health/Primary Care float team. The float team supports 11 JPS Patient Centered Medical Homes throughout Tarrant County.

Key Responsibilities
  • Collaborate with physicians to oversee and coordinate patient care
  • Review patient medical charts to assist with diagnosis and treatment
  • Make patient referrals as necessary
  • Monitor guidelines for care management and ensure compliance with JPS Health Network and Acclaim Physician Group standards
  • Provide care education to patients and families
  • Provide evidence-based and compassionate care to improve patient outcomes and satisfaction scores
  • Maintain a collaborative environment with providers, clinical staff, and support staff
  • Support academic, educational, and research endeavors
  • Attend Performance Improvement Programs and Peer Review committee meetings
Requirements
  • Master's Degree in Nursing from an accredited school of nursing or Graduate of an accredited Physician Assistant Program
  • Minimum of 2 years working independently as an advanced practice provider in a primary care setting
  • Current Texas Physician Assistant license or Texas APRN license with FNP certification
  • Valid, unrestricted DEA
  • Basic Life Support (BLS) certification
