Job Overview

As a key member of the Healthcare Recruitment Partners team, we are seeking an experienced Care Management Director to lead our efforts in developing and implementing strategic initiatives that drive patient outcomes and satisfaction.

Estimated Salary Range:

$120,000 - $180,000 per year, depending on experience and qualifications.

Key Responsibilities:

The successful candidate will be responsible for providing executive oversight of our care management operations, including strategy development, policy implementation, and quality improvement initiatives. This individual will work closely with our leadership team to align care management goals with organizational objectives, ensuring seamless integration across departments and sites.

  • Care Management Strategy Development: Collaborate with cross-functional teams to design and implement evidence-based care management strategies that improve patient outcomes and reduce healthcare costs.
  • Patient-Centered Care: Foster a culture of patient-centered care by promoting effective communication, coordination, and collaboration among healthcare providers, patients, and families.
  • Quality Improvement Initiatives: Develop and implement quality improvement projects that address identified areas for improvement, leveraging data-driven insights to inform decision-making.
Required Skills and Qualifications:

To succeed in this role, candidates must possess:

  • A Master's degree in Nursing (MSN) or related field;
  • At least 5 years of experience in care management, with a proven track record of success in leading high-performing teams;
  • Strong knowledge of clinical practices, regulatory requirements, and industry trends;
  • Exceptional leadership, communication, and interpersonal skills;
  • Able to think strategically, prioritize tasks effectively, and manage multiple projects simultaneously.
