About the Role

We are seeking a compassionate and experienced Nurse Practitioner to join our team at Abode Care Partners. As a key member of our integrated medical services team, you will have the opportunity to grow your medical skills while providing personalized care to seniors and patients with complex needs in their home, assisted living facility, or independent living community.

Responsibilities
  • Perform evidence-based medical care to ensure the highest quality of patient outcomes.
  • Examine and treat chronic and urgent episodic illnesses, utilizing your expertise to develop effective treatment plans.
  • Order and interpret laboratory and diagnostic testing to inform your medical decisions.
  • Collaborate with caregivers and other healthcare professionals to ensure seamless patient care.
  • Ensure patient medical records are kept up to date, maintaining accurate and comprehensive documentation.
Qualifications
  • Active state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • Active state DEA licensure.
  • Master's degree in Nursing (MSN).
  • Five or more years of nursing experience.
  • Strong interpersonal communication skills.
  • Proficiency in medical instruments and equipment.
  • Strong knowledge base of HIPAA and OSHA regulations.
  • Licensed driver with valid automobile insurance.
