Job Description

About the Position:

We are seeking a skilled Nurse Practitioner for Community Health to join our team at Cabarrus Health Alliance. This role is responsible for providing primary and preventive healthcare services to individuals and families in the community.

Responsibilities:

  • Conduct thorough nursing assessments to identify health needs and develop personalized care plans
  • Administer vaccines and perform minor office procedures as needed
  • Triage patients and provide education on healthy lifestyle choices
  • Collaborate with healthcare providers and community agencies to ensure comprehensive care
  • Document patient interactions and maintain accurate records

Requirements:

To be successful in this role, you will need a strong background in nursing with experience in public health or a related field. A current RN license in North Carolina is required, along with CPR certification. Excellent communication and interpersonal skills are also essential for building strong relationships with patients and colleagues.
